It\u2019s how it gets there.<\/em><br \/>\nEater\u2019s Digest, from <a href=\"https:\/\/fxnutrition.com\/\" target=\"_blank\" rel=\"noopener\">Replenish PDX<\/a> is one eater\u2019s reflections on the journey of her food.<\/p>\n<h2>this month: discussing digestion with a gastroenterologist<\/h2>\n<hr \/>\n<p>Christmas parties, Christmas eve, Christmas day and Boxing day. . .<br \/>\nHo ho ho, so much fun!<\/p>\n<h2>Yet sometimes the jolly times come with a not-so merry belly.<\/h2>\n<p>It may be the amount of food or the combinations of foods. It could be a bit more of the foods that you usually leave for special occasions (those special occasions just came one on top of each other these last couple of weeks).<br \/>\nOr, if you\u2019re among the <strong>60 to 70 million Americans <\/strong>that suffers from reported digestive distresses each year, this may, unfortunately, be a season not unlike any other.<br \/>\nRecently I had an open conversation with my friend <strong>Venu Julapalli M.D.<\/strong>, a gastroenterologist in Houston, Texas. I wanted to discuss how a medically trained doctor considers the role of food and nutrition in digestive health, especially given the nature of our upcoming <strong><a href=\"https:\/\/fxnutrition.com\/resolution\/\" target=\"_blank\" rel=\"noopener\">Resolution Detox<\/a><\/strong>, which allows you to sink into your own core health to kick off the new year.<br \/>\nAs you\u2019ll see, Dr. Julapalli had some questions for me too!<br \/>\n(You can read excerpts of our discussion below, followed by more details about Dr. Julapalli and the<strong> <a href=\"https:\/\/fxnutrition.com\/resolution\/\" target=\"_blank\" rel=\"noopener\">Resolution Detox<\/a><\/strong>.)<br \/>\nTo your happy belly in the new year.<br \/>\nWarmly,<br \/>\n<img decoding=\"async\" src=\"http:\/\/www.fxnutrition.com\/wp-content\/uploads\/2018\/03\/signature.png\" alt=\"Andrea Nakayama\" width=\"168\" height=\"55\" class=\"alignleft wp-image-9811\" \/><\/p>\n<hr \/>\n<p><strong>Digestive Wellness Discussion Between Andrea Nakayama &amp; Venu Julapalli M.D.<\/strong><\/p>\n<h2>diet does matter<\/h2>\n<p><strong>Andrea<\/strong>: Most of the time I am now finding that select physicians will work with what we\u2019re doing nutritionally with our clients and believe in it because there is more of an influx of a functional medicine approach which has a keen understanding of the power of diet. I mean, for a gastroenterologist to say diet doesn\u2019t matter is. . . I just don\u2019t understand that. <strong>There\u2019s this tube in your body that\u2019s feeding your cells,<\/strong> and how could it not matter? And I\u2019d love to ask you that question: how <em>you<\/em> see diet as a factor too. And if somebody\u2019s working with a physician that\u2019s saying diet doesn\u2019t matter, and they\u2019re in some sort of pain, they\u2019ve likely experienced on their own that diet <em>does <\/em>matter. . . that when they take something out of their diet they feel better, or they notice some symptom get worse after they eat a certain food, so that when their doctor says diet doesn\u2019t matter, they\u2019re likely starting to question their doctor. Do you find that to be the case?<br \/>\n<strong>Dr. Julapalli<\/strong>: Yeah, no doubt. First of all, saying diet doesn\u2019t matter. . . it\u2019s frankly absurd, and on a practical level, I would venture to say that 90% of physicians know that diet matters. They know this fundamentally. It\u2019s just a question of \u201cWhat am I going to do about it as a physician?\u201d So yeah, <strong>diet does matter.<\/strong> I get it. Obviously, <strong>everything that you eat is going to contribute to your health,<\/strong> but the physician either doesn\u2019t care \u2013 that\u2019s kind of the arrogant stance \u2013 or doesn\u2019t have the time or doesn\u2019t have the knowledge to point the patient in the right direction of how diet does matter. So, it\u2019s just not something the conventional medical physician is trained in.<br \/>\n. . . So, diet gets diminished. Yes, it\u2019s obvious that diet matters, and even the patient ultimately knows deep down that what they eat matters. It just gets back to \u201cYeah, I got it doc, but can you give me something to just fix it, and then I\u2019ll just continue to do what I can, and let\u2019s just fix the problem.\u201d<\/p>\n<h2>quick fixes vs. addressing the underlying problem sustainably<\/h2>\n<p><strong>Dr. Julapalli<\/strong>: . . . in the adult medical world. . . and probably similar things apply for kids with respect to their parents, but really, in adult medicine people want the quick fix, and they\u2019re much more likely to take a medicine in the faith and hope that that medicine is going to make their symptoms all go away fast.<br \/>\n<strong>Andrea<\/strong>: Exactly.<br \/>\n<strong>Dr. Julapalli<\/strong>: And quite frankly, I\u2019m also into quick fixes. I would love quick fixes if they\u2019re available. So, if there is a quick fix that can easily be done, I\u2019m all for it. But much of the time there isn\u2019t a quick fix for IBS (irritable bowel syndrome), or something, and so that kind of leaves me to my next question which is how soon would somebody expect to get or feel results because that\u2019s a nice motivator too? When you know or when you have faith that this isn\u2019t going to take a year necessarily, I may feel better pretty quick by making some of these fundamental changes.<br \/>\n<strong>Andrea<\/strong>: I\u2019m all for a quick fix too. Unfortunately, a lot of the quick fixes don\u2019t address the underlying problem so the challenge there is that the symptom may be alleviated, but the damage below is not. How is that (damage) going to crop up in different ways? Like, \u201cGreat. I don\u2019t feel that bloating anymore\u201d or whatever it is. . . \u201cI\u2019m pooping normally,\u201d but what\u2019s happening at an immune level, at a mental health level where those problems are not being addressed? (Especially since digestive troubles are so related to these other areas \u2014 immunity, mental health, skin issues and toxic overload.) So, I see it as a many pronged system of healing where there\u2019s no way that you and I eradicate each others\u2019 jobs. They go hand in hand.<br \/>\n<strong>If there\u2019s a need for a quick fix, don\u2019t assume it\u2019s addressing the cause of the symptom, which may be presenting elsewhere in the system.<\/strong><br \/>\nBut the answer to your question about how long does it take is really dependent on the individual. We see this all the time. People can come in and feel better in a week, or they can feel worse to begin with when you take them off of the inflammatory foods that are actually causing some opiate response and some adrenal response when they eat those foods, especially if they have a leaky gut \u2014 which I would venture to say most people in our modern day society do. And especially if they have any GI issues then they may have something that feels like a positive response to eating these foods that they\u2019re sensitive to and those will feel challenging to remove.<br \/>\n. . . So, it\u2019s a dance and a process for each individual that is really about finding their way. Because of that dance, if they don\u2019t feel better right away, they have to understand the <em>whys<\/em> and the <em>hows<\/em> of it, otherwise they\u2019re not going to stay engaged, precisely because it\u2019s not that quick fix.<br \/>\n<strong>Dr. Julapalli<\/strong>: I\u2019ll show a list of foods to be removed and it has all these things on there and invariably they patient will say, \u201cThat\u2019s everything I love.\u201d<br \/>\n<strong>Andrea<\/strong>: Right. Exactly. Yeah, it really is a dance with each individual and it involves helping them to find the smaller moments instead of, \u201cWell, everything\u2019s better now. It has <em>all<\/em> gotten better\u201d. At first it may be that <em>something<\/em> gets better. You know, we\u2019re always looking at some major factors to see how people are doing. Poop. Sleep. Those are some major factors that we\u2019re looking at, and if we can look at improving those first and foremost, so much starts to feel better in turn. That\u2019s how we all sustain motivation; through the small success. (And there\u2019s so much more to love than the foods that make us feel bad.)<\/p>\n<h2>digestive symptoms around the holidays<\/h2>\n<p><strong>Andrea<\/strong>: Are there times of year when you see or receive more patients with acute GI issues?<br \/>\n<strong>Dr. Julapalli<\/strong>: Yeah. I have noticed that, in particular with that. . . . (there are a couple of times a year including the autumn and then) I\u2019d say probably around the holidays. I think there\u2019s a lot of physiological issues that come into play. So, around the holidays I tend to see more of these chronic symptoms of upset stomach, irritable bowel-type symptoms.<br \/>\n<strong>Andrea<\/strong>: Yeah, it\u2019s interesting. I think (the holidays are) really tough for people, even if they have made dietary changes through our programs. First, they\u2019re not necessarily experiencing any symptoms that they\u2019re aware of because (of what I refer to as a) cloudiness between themselves and what they\u2019re feeling, and yet they do a detox with us and find that they feel so much better. But then they go into the holidays, and all of a sudden symptoms (they barely felt before) feel pronounced to them because they\u2019re actually more clear, the cloudiness has been lifted.<br \/>\nI like to remind people that sensitivity is a good thing. It means we\u2019re in communication as opposed to, \u201cWhy didn\u2019t I feel that before and now I do?\u201d<br \/>\nBut I see this happen a lot during the holidays because people feel deprived or they feel pressured by family. I\u2019ve been in that situation myself where it\u2019s, like, there\u2019s certain things I\u2019m not going to say <em>yes<\/em> to, but there might be that kind of middle ground and I don\u2019t want to make somebody feel bad and so I eat something that doesn\u2019t agree with me, and then therein starts the process (of sensitivity).<\/p>\n<h2>finding motivation<\/h2>\n<p><strong>Dr. Julapalli<\/strong>: I personally run up against that. The other major thing that I will run up against as a challenge is it\u2019s that human challenge of how do you help somebody? It\u2019s not really even about motivation or willpower. How do you help somebody see a different way to live? How do you end up doing that? I mean, we go through this in the house, me and my wife!<br \/>\nThis is a human dilemma: <strong>you may intellectually or cognitively know what to do or how to go about doing things, but that\u2019s not enough.<\/strong> That\u2019s not even close to being enough.<br \/>\nTo me, that\u2019s where the rubber meets the road. I mean, how do you believe people get motivated to do this when their habits are ingrained, they don\u2019t see that this is going to connect to feeling better when their whole identity \u2014 you know, the way they view themselves, their relationships with their families and friends is built around a certain lifestyle. When your family or friends, when they\u2019re starting to eat around you during the holidays, for example, and they\u2019re seeing you trying to do something else, they say \u201cWhat\u2019s the big deal?\u201d That\u2019s really where the rubber meets the road. How does that actually play out?<br \/>\n<strong>Andrea<\/strong>: Yeah, I think that\u2019s where our work really comes into play. The first part of my answer to that would be education, which is why I think we make a difference in peoples\u2019 lives because we\u2019re really educating as we\u2019re changing. So, we\u2019re helping people to understand <em>why<\/em>; not just telling them what (which food) has to come out, this needs to be removed, period. We\u2019re really explaining <em>why<\/em>, and I think that\u2019s my passion and my love. (I also believe in it\u2019s success in fueling motivation.)<br \/>\nBut I also know for a fact that people need to have a very strong, motivating factor to make dietary change and things like weight loss or \u201cI\u2019m on a detox\u201d aren\u2019t (necessarily) big enough to make sustaining change. What is big enough to make sustaining change is a pain point, and that pain may be a physical pain, but to be honest, I see that not to be the case with adults; that\u2019s more so with children. If children have a physical pain when they eat something, they\u2019re done with that food. They make that association really quickly. Adults are much more caught up in that social component that you\u2019re talking about \u2014 you know, the emotions that come up when we eat something, or making sure that we\u2019re not inconveniencing somebody else, whether it be a family member, friends, or even a waiter!<br \/>\nThere are all these aspects associated with food that make it very difficult to change our diets, and so that\u2019s where there\u2019s a lot of work to be done to tap into what is the motivating factor for somebody to make dietary change and stick with it. And that is the piece that you and I are passionate about.<br \/>\n<strong>It goes beyond the physical signs or symptoms, and it goes into really what do you want in your life and making those connections.<\/strong><br \/>\nFor a lot of people, one of the simple things we see is \u201cI want to feel like myself again.\u201d Especially for women \u201cI want to feel like myself again\u201d is something people say to us often. What is it that we each want to be doing\/achieving\/feeling in our lives, and how is it that <em>that<\/em> becomes a motivating factor for each of us so that the dietary change goes well beyond the physical symptoms, whether they be discomfort or added weight. (It\u2019s ultimately a vehicle to something much bigger.)<br \/>\nThis is why in our online programs, even with hundreds of people are in them at a time, one of the most important things for us is our message board or support forum. I\u2019m there everyday and I\u2019m writing about the practical and the tactical. And then there\u2019s a lot of motivational writing on there as well. So, when somebody\u2019s struggling with what\u2019s going on for them, that\u2019s where I\u2019m really coming in with the whole different perspective to support them (perspective beyond the mere tactics of what to do.) And other detoxers are going to chime in with their support as well! It becomes a real community of making this change <em>work<\/em>, and eliminates that feeling of loneliness that can happen when you show up at a party and you don\u2019t eat like everyone else in your community.<br \/>\n<strong>To stay motivated and make sustainable change we need both mentors and community.<\/strong><\/p>\n<h2>what is Resolution Detox<\/h2>\n<p><strong>Andrea<\/strong>: The idea for <a href=\"https:\/\/fxnutrition.com\/resolution\/\" target=\"_blank\" rel=\"noopener\">Resolution Detox<\/a> is that we start off our year focusing back on our digestion after we\u2019ve maybe been off-track from the holidays, and feeling it in our systems, and wanting to start the new year right.<br \/>\nAnd I\u2019ve also been researching the concept of <em>Resolution<\/em>. There are some really nice studies that show that people who make New Year\u2019s resolutions actually are more successful in their pursuit in everything because they\u2019re making a commitment to themselves. There\u2019s no longer the unhappiness that (not only discomfort but) ambiguity can bring. There\u2019s a whole trend in our society to move away from resolutions. Like, it\u2019s not good, it\u2019s not a good thing, it just leads to failure.<br \/>\n<strong>But the research actually shows otherwise; that making resolutions leads to success.<br \/>\n<\/strong><br \/>\n<a href=\"https:\/\/fxnutrition.com\/resolution\/\" target=\"_blank\" rel=\"noopener\">My question for everyone is this: How can you start your new year off on a positive foot just by slowing down for ten days, and focusing in on your core health ~ the health that is at the root of all other health issues ~ and then move from there into your new year with some baseline success already in place?<\/a><\/p>\n<hr \/>\n<p><em>Dr. Venu Julapalli is founder and president of Integral Gastroenterology Center, P.A., an active private practice in gastroenterology in Houston, Texas.
